First of all Log in facebook account and Go to you facebook page / fan page follow Instruction

When you enter in you facebook page  then click on setting tab in top of you facebook page 

when you enter the setting page now in this page see the blow in the option (delete your page ) is disable click the front of  (delete you page ) edit tab 

when you click on the edit tab now on this page the option (delete your page ) is convert in "permanently delete (page name)" now click on this option "permanently delete (page name)"
